Available immediately is an exempt, salaried position to directly provide Individual service coordination, provide home visiting services to provide expectant and new parents with the opportunity to receive the education and support they need at the time their baby is born.

Job Title: Lead Home Visitor

Program: Healthy Families America

Reports to: Healthy Families America Program Director

Position purpose: The HFA home-visiting model promotes early nurturing parent-child interactions as the foundation for life-long healthy development in children. The services place emphasis on working with the expectant and new parents to improve health outcomes by increase in access to primary medical services, reduce child-maltreatment, and improve family self-sufficiency. The Lead Home Visitor is a unique position that provides services to families and supervision and training to new hires. The Lead home visitor is a positive role model within the team, supports team compliance with policies and procedures, attends designated meetings and represents the HFA program in the absence of the Program Director

Primary Responsibilities

Under the direction of the Program Director, provided clinical and administrative supervision, training and job development to the staff.

Monitor documentation and data entry compliance.

Effective and timely communication of program changes/updates to staff

Demonstrate understanding relevant federal, and state legislative regulations, Ohio Administrative Code, policies and procedures that govern home visiting services, mandated reporting, HIPPA program compliance, confidentiality and ethical standards of care.

  • Develop Family Goal Plans in collaboration with families
  • Utilize developmental screening instruments for infants and toddlers birth- to-three
  • Engage families as active team members in the screening, developmental evaluation, ongoing assessments and intervention processes.
  • Coordinate service appointments based on family needs/preferences in collaboration with other services providers
  • Coordinate and monitor delivery of services with families and providers
  • Link parents and children to appropriate resources in Franklin County
  • Assess the appropriate match between the expressed needs and interests of referrals and the services available through HFA
  • Assure that perspective participants have a clear understanding of the services available and the organization’s expectations surrounding their participation.
  • Be knowledgeable of the programs and services of The Center for Healthy Families and resources it makes available to pregnant and parenting teens
  • Assist enrolled teens and their families in managing crisis situations, advocating and coaching as required
  • Maintain communication with both partner and non partner organizations providing services to participants to monitor progress and ensure appropriate service delivery that contributes to achievement of goals

Manage and operate an administrative process and tool that tracks entrants into and through Healthy Families America.

  • Understand and enforce appropriate privacy, security, and privilege restrictions when accessing and sharing data
  • Track complete and relevant client and service data
  • Produce reports
  • Participate in ongoing evaluation of HFA program to maximize effectiveness and impact

Develop and maintain effective working relationships between The Center for Healthy Families and partnering organizations, area social service agencies, and other select entities within the Franklin County community.

  • Seek input, advice, and feedback from partnering organizations to assist in performing position responsibilities
  • Serve as a liaison to assigned committees of The Center for Healthy Families
  • Provide administrative support to assigned committees of The Center for Healthy Families

Perform other duties as assigned.

Minimum Requirements

Education:

  • Bachelor’s degree in Social Work or related field
  • LSW, PC or MFT
  • Experiential knowledge of social service agencies, and/or health care institutions, and/or community organizations
  • Some schedule flexibility, defined as infrequent night or weekend hours

Experience

  • 5 years’ experience directly serving clients or consumers, both conducting an assessment and providing a responsive service either directly or through referrals
  • Significant work experience with teens in providing individual and group counseling.
  • Experience or knowledge of documentation requirements for Medicaid as well as Federal, State and/or county funding.

Preferred Qualifications

Education:

  • Master’s degree, preferably in social work or related degree.

Experience:

  • Significant employment, volunteer, or personal experience with teens
  • Leadership experience, or demonstrated interest in leadership
  • Minimum of 2 years work experience in a position requiring an MSW or LSW
  • Bi-lingual (English and Spanish)
