Our Approach

The Center for Healthy Families, working in an innovative partnership with public and private agencies throughout Franklin County, is integrating and expanding existing community-based services in a coordinated system that increases accessibility and utilization of existing services, reduces duplication and overlap of programs, and improves overall efficiency and effectiveness of community resources.

Two core components make up the pillars of sustainability of the CHF Model. These are the Advocacy, Research and Education Center and the Healthy Families Connection Collaborative.

ADVOCACY, RESEARCH AND EDUCATION CENTER

A partnership with area research institutes collects and track data related to teen pregnancy and key indicators of change. Information will be used to inform public policy and educate community members and public officials on best practices and evidence based strategies both for preventing teen pregnancy and for improving the health, self-sufficiency and well-being of teen parents and their children. We also work with these partners to evaluate the measurable outcomes of our work.

HEALTHY FAMILIES CONNECTION

Healthy Families Connection is our extensive network of community partners that work together to develop individualized, holistic goal plans for teens and their families. The goal plan is the “road map” for how each partner agency will work with the teen parent over an 18-to-24 month period. Five service components are offered as core to our network, with customized support service as needed. The Center for Healthy Families’ network consists of existing organizations that have stellar reputations, with proven results in the areas of:

  • Positive Parenting
  • Mentoring
  • Leadership Development (Education, Financial Literacy, Self-Esteem, Employment)
